1. A recent passport-sized photograph.
2. Means of Identification - this could be any of the following stated below:
i.Valid International passport
ii.Valid Driver's Licence
iii.Permanent Voters Card
iv.National ID Card or slip
v.Notarized National ID for Non-resident
3. Proof of Address - this could be any of the following stated below:
a. Valid utility bill in your name within the past 3 months(i.e. a pre-paid meter ticket,electricity bill, water bill, waste bill, land use charge or fixed-line telephone bill)
b. Tenancy agreement with valid rental receipt
4. Signature specimen - image or document.
Please note that the maximum size for each document or image should not exceed 2 megabytes (MB).
